Hyaluronic Acid (HA) Filler

Hello. I have had Hyaluronic Acid (HA) fillers injected and dissolved in my nasolabial folds multiple times. About four years ago, I also used a collagen booster called Juvelook, which caused no issues at that time. However, recently, after getting HA fillers, my face looked unnatural, so I tried dissolving and re-injecting them, but the area around my nasolabial folds remained unnatural. Since then, I have attempted to dissolve them about five times. I even underwent a procedure where the target area was precisely identified using ultrasound, injected with hyaluronidase, and then manually expressed (Pinhole Extraction), but the granules remain in my nasolabial folds and I can still feel the filler. Is it possible to improve this? When I had an ultrasound, they told me that the filler is distributed across both the deep layers and the muscle layers.

Hello, based on your description, I believe there is still an opportunity for further improvement. The **lumps/nodules** formed by Juvelook are quite different from residual **Hyaluronic Acid (HA)**. While **HA** can be treated with hyaluronidase, the PDLLA particles, nodules, or surrounding fibrotic tissue formed by Juvelook cannot be directly dissolved by hyaluronidase. Although you have previously undergone **Pinhole Extraction**, the extraction techniques, **ultrasound** positioning, depth of entry, and the approach to handling residual **lumps/nodules** can vary significantly between physicians. Therefore, having had an extraction procedure before does not necessarily mean there is no room to address the remaining **lumps/nodules**. In our clinical practice, we frequently encounter patients who have already undergone dissolution or extraction elsewhere but still have residual particles, **lumps/nodules**, or unevenness. We have extensive experience in **ultrasound-guided** minimally invasive extraction for these types of residual **lumps/nodules**. Especially since you mentioned that **ultrasound** shows the filler has distributed into the deep layers or even the muscle layer, this situation requires careful, layer-by-layer verification of the location; it cannot be resolved simply by injecting more hyaluronidase. If you have current **ultrasound** images and photos from before and after your treatments, please feel free to provide them for our assessment. We will first determine whether what you are feeling is residual **HA**, Juvelook nodules, or fibrotic tissue, and then evaluate which areas have the potential for further improvement.
