She had autologous fat grafted into the hollows of her upper eyelids at the end of 2023. About three months later the upper eyelids began to look clearly puffy, and after she gained around 5 kg the bulge became more obvious. At this point, three thoughts usually come up: will it go down if I wait, can it be dissolved with an injection, and should I just have it all taken out at once? Waiting will not necessarily help. Grafted fat that survives is fat tissue; it does not fade with time, and it can grow along with weight gain. Dissolving has no matching drug. Hyaluronidase only acts on hyaluronic acid and does nothing to fat. Taking it all out at once is a high-risk option in the upper eyelid. The skin is very thin, the lacrimal gland sits at the outer upper corner and the eyeball lies beneath, so taking too much or too little both show; take too much and a bulge becomes a hollow. So the first step is to see clearly. Ultrasound confirmed the fat sat superficially. Superficial fat has nothing in front of it, so its outline shows at rest and more clearly when the eyes are squeezed shut. Only once the layer is confirmed does removal come up, and it is staged: conservative the first time, then, after full healing, a second session only if something still stands out. She made one more decision herself: the right eye first, the left after seeing the result. That gave the after photo a comparison of its own: same day, same light, same body weight, with the right eye treated and the left eye not yet treated. At the one-month visit the right upper eyelid was much smoother, and after discussion no second session is planned for now. One month is still within recovery; the final result is judged at around three months.
