Tear Trough and Under-Eye VIVABELLA and Hyaluronic Acid Lump Case: Ultrasound Evaluation and Pinhole Extraction from 0.5 cm to 4 cm
Dr. Ta-Ju Liu · 11/7/2026
When she came in July, there was only a small lump of about 0.5 cm near her inner canthus and tear trough (she had previously received VIVABELLA and hyaluronic acid). I suggested extracting it, but she said she wanted to go home and think about it.
Four months later, she returned, stating she had undergone an enhanced treatment. Both under-eye areas had coalesced into a large mass, measuring 3 to 4 cm each, feeling as hard as a stone, to the extent that her lower eyelid could not even be pulled down. On ultrasound, it had already evolved into the most difficult category of filler lumps to manage: as hard as stone and located very superficially.
Not all under-eye lumps can be dissolved using simple methods; using the wrong method often makes them harder, more active, and larger. It is not about avoiding treatment, but rather avoiding casual attempts: first, assess clearly with ultrasound, then extract through a single pinhole. Recovery after extraction will be shared at the follow-up visit in two to three months.
