At age 25 this patient received a collagen-stimulator nasal augmentation. A palpable lump appeared on the right side of the nose soon afterwards and persisted for 15 years. Over the years she consulted several physicians and received conflicting advice — some suggested living with it, some said surgery was needed, and one told her "there is nothing there" — yet she could always feel it herself. High-frequency ultrasound now clearly demonstrated a hypoechoic lesion on the right side of the nasal bone, while the left side was clean — matching exactly the location she had described for years. Imaging confirmed the residual material was real, not imagined. Collagen stimulators (PCL / PLLA / CaHA types) have no corresponding dissolving enzyme and cannot be injected away; when they persist or form a firm nodule, ultrasound-guided physical removal is the most direct option. After individualized evaluation, ultrasound-guided minimal-incision extraction has been scheduled. (Shared with the patient's consent; images de-identified.
